Request a Visit
Contact us by phone, email, or through our appointment request form to schedule your telehealth visit. We will confirm your appointment and send connection instructions.
Receive Your Link
You will receive a secure, encrypted video link before your appointment. No special software is required — visits are accessible from any smartphone, tablet, or computer.
Meet with Our Team
Connect with our DNP or RN at your scheduled time. Your session is private, confidential, and conducted over a HIPAA-compliant platform.
Follow-Up & Coordination
After your visit, our team follows up with any care coordination, referrals, or documentation needed. We keep your support coordinator and care team informed as authorized.

Who Can Use Telehealth
Individuals Currently Enrolled
Individuals receiving HCBS waiver services from Siebiti Wellness Center can access telehealth for nursing consultations, health check-ins, and care coordination.
Family Members & Guardians
Family caregivers and legal guardians can schedule telehealth consultations to discuss clinical questions, medication concerns, and care planning.
Prospective Clients
Individuals and families exploring services can meet with our clinical team via telehealth for a no-cost pre-admission consultation and intake discussion.
Support Coordinators & Referral Partners
Support coordinators and referral partners can connect with our team via telehealth to discuss client needs, service capacity, and the intake process.
Coverage
Telehealth Billing & Coverage
Telehealth visit billing depends on your enrollment status and the nature of the consultation.
- Telehealth visits for currently enrolled waiver clients may be covered under the DC DDA HCBS Medicaid Waiver
- Pre-admission and intake consultations are offered at no charge for prospective clients
- Family and caregiver guidance sessions are available on a private-pay basis
