DC DDA HCBS Waiver Services
Person-Centered Supports for Independent Community Living
Siebiti Wellness Center delivers individualized, person-centered residential and in-home supports under the DC DDA Home and Community-Based Services Medicaid Waiver Program for adults with intellectual and developmental disabilities in Northeast Washington, DC and across the District.
Individuals and families have the right to choose their own providers. Siebiti Wellness Center does not accept referrals directly from DDS — all placements are made through the individual's Support Coordinator based on assessed needs, preferences, and Individual Support Plan goals.
Supported Living
24/7 or scheduled residential supports in the home of the individual's choice.
Our Supported Living program provides comprehensive, around-the-clock or scheduled residential supports for adults with intellectual and developmental disabilities who live in their own homes or apartments in Washington, DC.
Service Features
- Personal care assistance: bathing, grooming, dressing, and hygiene
- Medication administration and health monitoring under nursing supervision
- Meal planning, grocery shopping, and cooking support
- Household management: cleaning, laundry, and home safety
- Community access: appointments, recreation, worship, and social activities
- Transportation coordination and accompaniment
- Crisis prevention, de-escalation, and emergency response
- ISP goal implementation with documented progress notes
Supported Living Periodic
Flexible check-in and on-call supports for individuals living more independently.
Supported Living Periodic services are designed for individuals who live with greater independence but benefit from regular staff contact, skill reinforcement, and on-call availability.
Service Features
- Scheduled wellness check-ins — daily, weekly, or as authorized by ISP
- Skill reinforcement for budgeting, cooking, and self-care routines
- Medication management reminders and health monitoring
- On-call staff availability for questions, concerns, and non-emergency situations
- Crisis prevention planning and early intervention
- Connection to community resources, social activities, and natural supports
- Regular communication with Support Coordinator and family as authorized
Residential Habilitation
Structured daily programming that builds adaptive skills and community participation.
Our Residential Habilitation program delivers structured, goal-directed habilitative programming within the home setting, focused on building and maintaining the adaptive living skills that support greater independence.
Service Features
- Activities of daily living (ADL) skill-building: cooking, cleaning, personal hygiene
- Instrumental ADL training: budgeting, shopping, using public transit
- Communication and social skills development
- Health and wellness education: nutrition, exercise, and self-advocacy
- Community integration activities: libraries, recreation centers, faith communities
- Structured behavioral support aligned with positive behavior support plans
- Monthly progress reviews and ISP goal tracking
- Delivered in HCBS Settings Rule-compliant, integrated residential settings
In-Home Supports
Direct support in the individual's home — personal care, health, and daily routines.
In-Home Supports provide direct, hands-on assistance within the individual's own home, covering personal care, health management, household tasks, and daily routines.
Service Features
- Personal care: bathing, grooming, dressing, toileting, and mobility assistance
- Medication reminders and health monitoring (vitals, wound care, chronic condition management)
- Meal preparation and nutritional support
- Light housekeeping, laundry, and home organization
- Accompaniment to medical appointments and community activities
- Coordination with healthcare providers, pharmacies, and specialists
- Documentation of daily activities and health observations
- Nursing oversight and clinical consultation available for complex health needs
Respite
Planned and emergency relief for family caregivers — in-home or community-based.
Respite services give family caregivers the time and space to rest while their family member receives consistent, safe, and engaging support from our qualified staff.
Service Features
- Planned respite: scheduled in advance for vacations, appointments, or regular caregiver breaks
- Emergency respite: short-notice availability for caregiver illness, family crisis, or unexpected need
- In-home respite: staff come to the individual's home, maintaining familiar routines
- Community-based respite: supervised outings, recreation, and social activities
- Consistent staffing to minimize disruption and maintain trust
- Health monitoring and medication management during respite periods
- Detailed handoff communication with family caregivers before and after each respite period
Telehealth Services
Clinical Care — Wherever You Are
Siebiti Wellness Center offers secure, HIPAA-compliant telehealth visits with our nursing team for individuals we serve, their families, and prospective clients. Our nurse practitioners and registered nurses provide remote clinical consultations, health assessments, and care coordination — reducing barriers to access while keeping your health information private and protected.
All telehealth sessions are conducted over HIPAA-compliant, encrypted platforms. Your health information is never shared without your consent.
Nursing Consultations
Schedule a virtual visit with our DNP or RN team for health assessments, medication reviews, chronic condition monitoring, and clinical questions.
Care Coordination Support
Our nursing staff can coordinate with your primary care physician, specialists, and pharmacies remotely.
Family & Caregiver Guidance
We offer telehealth consultations for family caregivers and guardians — answering clinical questions and providing guidance on managing complex health needs at home.
Pre-Admission Assessments
Prospective clients and their support coordinators can meet with our clinical team via telehealth to discuss service needs and begin the intake process.

Payment & Insurance
How Services Are Funded
Siebiti Wellness Center's HCBS waiver services are funded through the DC Medicaid system. We work closely with individuals, families, and support coordinators to ensure a smooth authorization and billing process.
DC DDA HCBS Medicaid Waiver
All five of our core services are funded through the DC Department on Disability Services (DDS) Home and Community-Based Services Medicaid Waiver Program. Eligible individuals must be enrolled in DC Medicaid and have an active Individual Support Plan (ISP) authorizing the specific service.
- Services billed directly to DC Medicaid — no out-of-pocket cost to eligible individuals
- Authorization required from DDS and the individual's Support Coordinator before services begin
- Billing managed entirely by Siebiti Wellness Center — families do not handle claims
- Annual ISP review determines authorized service hours and funding levels
- We assist families in understanding the authorization process and timeline
Private Pay Options
For individuals who are not enrolled in the DC DDA HCBS Waiver or who wish to supplement waiver-funded services, Siebiti Wellness Center offers private-pay arrangements.

Telehealth Visit Billing
Telehealth consultations with our nursing team are available to individuals we currently serve, their families, and prospective clients. Billing depends on enrollment status and the nature of the consultation.

- Pre-admission and intake consultations are offered at no charge for prospective clients

Nursing Student Preceptorship — FMP & Psych
Siebiti Wellness Center offers structured preceptorship placements for nursing students in both Family-Medical-Pediatric (FMP) and Psychiatric-Mental Health tracks. Under the direct supervision of our DNP and RN clinical team, students gain hands-on experience in community-based care for adults with intellectual and developmental disabilities.
- FMP preceptorships: clinical rotations covering personal care, medication management, chronic condition monitoring, and community health
- Psychiatric-Mental Health preceptorships: exposure to behavioral support, de-escalation, mental health co-occurring conditions, and trauma-informed care
- Supervised by a DNP-prepared preceptor with community health and IDD specialty experience
- Placements available for BSN, MSN, and DNP students — contact us to discuss your program's requirements
- Affiliation agreements available for nursing schools and university programs
- Students work directly with individuals served under active ISPs in real community settings
- No cost to students — contact us to inquire about current placement availability
A Note on Private Health Insurance
Siebiti Wellness Center's HCBS waiver services are funded through DC Medicaid — not private health insurance plans. If you have questions about whether your private insurance covers any supplemental services, our team can help you understand your options.

How We Deliver Services
Nursing-Led Clinical Oversight
Every individual served by Siebiti Wellness Center has access to nursing oversight led by our DNP and RN team.
Trained, Screened Direct Support Professionals
Our DSPs complete DC DDA-required training, CPR/First Aid certification, and background screening before working with any individual.
Person-Centered, ISP-Driven Planning
We participate actively in each individual's ISP planning process. Supports are built around what is important to and important for each person.
Quality Management & Incident Review
We maintain a robust quality management system including incident reporting, root-cause analysis, corrective action planning, and regular internal audits.
