Siebiti Wellness Center is committed to upholding the rights of every person we serve. This page describes your rights as an individual receiving services and how to raise a concern or file a grievance.
Your rights as an individual receiving services
Every person served by Siebiti Wellness Center has fundamental rights protected under DC law, the DDA HCBS Medicaid Waiver, and our agency policies. These rights apply regardless of the services you receive, where you live, or the nature of your disability.
Dignity and respect
You have the right to be treated with dignity, respect, and courtesy at all times by all staff and agency representatives.
Privacy and confidentiality
Your personal information, health records, and service details are confidential. Information is shared only with your consent or as required by law.
Person-centered services
You have the right to services that reflect your individual goals, preferences, cultural background, and choices — not a standardized program.
Freedom from abuse, neglect, and exploitation
You have the right to be free from physical, emotional, sexual, and financial abuse, neglect, and exploitation. All staff are trained and screened to uphold this right.
Participation in your own planning
You have the right to participate in developing, reviewing, and modifying your Individual Support Plan (ISP). You may bring a family member, advocate, or support person to any planning meeting.
Informed consent
You have the right to receive clear information about your services, to ask questions, and to give or withhold consent before any service begins or changes.
Access to records
You have the right to review your own service records. Requests are processed within 10 business days.
Freedom of association and community access
You have the right to associate with people of your choosing and to participate in community activities, employment, and civic life.
Right to file a grievance without retaliation
You have the right to raise concerns, file a grievance, or contact external oversight agencies without fear of retaliation, punishment, or loss of services.
How to file a grievance
If you believe your rights have been violated, you are dissatisfied with your services, or you have a concern about how you have been treated, you have the right to file a grievance. We take all grievances seriously and respond promptly.

Raise your concern
You may raise a concern verbally or in writing with any Siebiti Wellness Center staff member, your Direct Support Professional, or the Quality Management Director.

Formal grievance submission
To file a formal grievance, contact our Quality Management Director, Fatmata Nellay LPN, by phone at 202-438-5017 or by email at [email protected]. You may also submit a written grievance by mail to 1503 Evarts Street NE, Washington, DC 20018.

Acknowledgment within 3 business days
We will acknowledge receipt of your grievance within 3 business days and provide you with a written confirmation of the grievance and the expected timeline for resolution.

Investigation and resolution
We will investigate the grievance and provide a written response within 15 business days. If more time is needed, we will notify you and explain the reason for the delay.

Appeal
If you are not satisfied with the resolution, you may appeal in writing within 10 business days of receiving our response. Appeals are reviewed by agency leadership and resolved within 15 additional business days.
External oversight and advocacy resources
You always have the right to contact external agencies. Filing a grievance with Siebiti Wellness Center does not prevent you from also contacting any of the following:
DC Department on Disability Services (DDS)
The state agency that oversees HCBS Medicaid Waiver providers in Washington, DC.
DC Office of the Long-Term Care Ombudsman
Advocates for the rights of individuals receiving long-term care services in DC.
DC Office of Human Rights
Investigates complaints of discrimination in DC.
DC Disability Rights Advocacy
Provides legal advocacy and protection for people with disabilities in DC.
