Self-referral for individuals and families
If you or a loved one has an intellectual or developmental disability and is enrolled in the DC DDA HCBS Medicaid Waiver, you can contact us directly to inquire about services and availability.

Initial conversation
We will speak with you or your family member to understand your needs, goals, and the services you are interested in.

Coordinate with your support coordinator
Your DDA Support Coordinator will need to authorize services. We work directly with your coordinator to complete the necessary paperwork.

Service agreement and start date
Once authorization is in place, we finalize your Individual Support Plan and schedule a start date that works for you.
Provider referral for support coordinators
We welcome referrals from DDA Support Coordinators, case managers, hospital discharge planners, and other provider agencies. We respond to all referrals within one business day.

Our clinical team reviews the referral and confirms service availability. We will contact you within one business day to discuss fit and next steps.

We work with you to gather required documentation: ISP, medical records, and DDA service authorization. Our team handles the paperwork efficiently.

Transition and onboarding
We coordinate a smooth transition, including a home visit, staff introduction, and Individual Support Plan review before services begin.

Referral information checklist
To process a referral quickly, please have the following information ready. You do not need all items to make an initial contact — we can gather details together.
- Individual's full name and date of birth
- DDA waiver type and current authorization
- Services requested (e.g., Supported Living, In-Home Supports, Respite)
- Current living situation and any transition timeline
- Support coordinator name and contact information
- Most recent Individual Support Plan (ISP)
- Relevant medical history or behavioral support needs
